For all upcoming scheduled courses you may download the registration form directly from the course page and fax it to the Course Coordinator to secure your spot. You need to either type or print the necessary information including the City, Location & Dates. After you have submitted this fax, our Educational Program Director will be in touch with you shortly thereafter to confirm your registration.
